本人經醫生診斷開葯情況下,已服用上述兩種葯超過兩年,尤其是Zoloft幾乎三年來未停過服食,我知道它們都是鎮靜劑,Dogmatil Forte會影嚮女性經期週期,引發暗瘡.
因本人將有一年時間需服食該兩種葯物,請問長期服食此兩種葯物對身體有何不良影嚮或該兩種葯物有何其他副作用。


嚴格來說這些是抗憂鬱藥,你說的副作用都是對的,但如果你巳服用了一年而沒有其他的問題,就應該不用太擔心了。
任何精神該藥物都不應長期服用,一來影響肝腎功能,二來影響精神和情緒。如果情況許可的話和病情已經安定下來,就應該按步按照醫生指示減少服用劑量。當然也有病人長期服用。

I am a IBS patient and currently taking a drug call "smecta"
(Dioctahedral Smectite 3.000g +  Excipients q.s. for a sachet weighing 3.760 g ). This is a powder form drug and need to put into water first.
I would like to know more about this drug, how it cure IBS problem, is this samiliar to antidiarrheal drugs? etc.

Basically the drug is working as an absorbent, to absorb the excessive water
and toxin in the gut to prevent diarrhoea.
Its effect is mild, but very safe.

全身麻醉

還記得我上次向您請教過暗瘡水一事嗎?
我已用了多個月了,效果顯著.
現有一事相求,小兒要在七月底做割包皮手術,
他今年快九歲了,而且還患有哮喘,醫生建議到時用全身麻醉,
在此情況下,我有何事項特別要注意呢?手術前後還有甚麼要留意?

你可以在藥物新聞組,可能有醫生能夠幫助你。
最主要的是在全身麻醉以前,遵循指導,如禁食,這減少在麻醉嘔吐的可能。如果你的儿子患有哮喘,你必須告訴你的醫生他在服用的藥物,尤其口服類固醇,因為它可能在麻醉中或后引起突然的血壓下降。

鼻敏感藥
你好 . 客氣話不多說

幾個月前..突然在每晚 5:00am - 7:00 am 時候總會在睡眠狀況裡被....
溜鼻水和打噴嚏弄醒...很是擾亂,最慘是睡我旁邊的無辜者被我害得無心睡眠
看了很多個醫生.../她們話我鼻敏感吃掉很多種藥..都無效
最後,,連中醫都看上,,,也沒有辦法
連土法, Salina 洗鼻, 也不成功總是在好夢正濃的時候,,,
被迫起床.......溜鼻水和打噴嚏

有日, 在藥房..介紹我吃 Kestine10mg tabs ebastine
早晚一粒.果然有效,但,我已連續吃上6星期了如果不吃,又故態復萌,無覺好睡..

現想請教
這藥物,有什麼副作用??
長期吃,有無害呢??

請給我一些意見

Kestine 是一類比較新的抗過敏藥,沒有暈的副作用。
一般使用是很安全的。
但是它不能和一些抗生素一同使用,你最好告訴你的醫生你在使用這藥物。
鼻敏感差不多沒有根治的方法,也許改變居住和工作的環境有幫助。

Rituximab

Dear Wong  chemist 
Thank you very much . you are right  I am a cancer ( blood) patient in  XX H,  now I  still on chemotherapy used  the drug .is FLUDARABINE. . but after  used 9 times Fludara my DR  suggest  me  to try the new drug on  march  but I  know nothing about this Riluximab .I only know it is very  very expensive .so  can  you found some thing about this drug information  for me . I also know  this drug in U S A  for some C A patients used. in H K .I think  I am the one ( white rat ) so please give me a big  help O K ?  I am waiting  for you mail.  thanks.

碘放射治療

多謝黃藥師專業知識.但我並不是有大頸泡的情形出現. 
據聞此種放射治療會有後遺症,不但會不孕並且會有機會返發. 
可否再請問下: 
1)
除此種"碘-放射治療"外,有冇其他更好的方法呢? 
2)"
碘放射治療"是否很普通及安全的一種方法呢?

如在美國或歐洲....etc )

What is "Couch Grass" and it's function? (I've found it from an nutritional product)

Basically it is a diuretic, i.e. to encourage passing urine, that has been used in treatment of various kidney diseases traditionally.

Cozaar
The medical tablet name:"Cozaar",
this is for Nephrology. How about the major strength x major weakness

Cozaar  is a new drug known as Angiotensin-II receptor antagonist, which contain losartan 25mg or 50mg per tablet, for the reduction of blood pressure.

Major advantages:
Unlikely other similar therapy, it does not appear to induce persistent cough.
Relatively mild side effects when it is compared to other hypertensive medications.

Major disadvantages:
Very expensive.
New drug, population safety profile has not been fully established.
Should not be used  in patients with some forms of renovascular(kidney-blood supply) diseases that affect both kidneys (please check with your doctor, as you mentioned "Nephrology")

 Is it possible to purchase a small supply of Clonazepam through your company? I am only requesting 30 .5mg for personal use because of apersonal crisis.  Thanks for your help.

However, I believe your doctors would surely prescribe the Rivotril (clonazepam 0.5mg in UK) to you if you are having epilepsy problem(its main indication).



Cimetidine

The patient had a history of chronic gastric ulcer and takes cimetidine for his therapy.But some times later the diagnosis show that he have deep vein thrombosis and pulmonary embolism,should this patient continue to use cimetidine or change to other drug?

(It is one of your case studies?! You should do your own work!)
Cimetidine does not directly affected the thrombosis and pulmonary embolism, but it would certainly interact with the other drugs may be employed to treat the embolism/thrombosis or the cardiovascular problems that caused them.
The only reason of using cimetidine, but not the other H2 antagonist is its price. Newer H2 antagonist, such as ranitidine(Zantac), famotidine (Pepcidine) which do not inhibit the cytochrome p450 metabolic enzyme would offer much fewer interactions with other drugs.
For a long term therapy, the patient should be tested to see whether H. Hylori is the cause of the chronic ulcer. If that is the case, proton pump inhibitor in combination with antibiotic should be used to eradicate the ulcer causing bacteria once the patient has stablised on his/her cardiovascular condition.
That's my opinions, what's yours?
 

Inderal

I am 26 and had been on inderal La since september of this year.  My Doctor cut my dosage down to 60 mg and two weeks later I was taken off of the medication.  Is this harmful, is there a risk of withdrawal symptoms such as chest pain etc..I have been on Inderal for high blood pressure. It is back to normal, but will it return? thank you for taking the time to read my concern.

Inderal LA is a long acting form of "beta-blocker", that can slow down for heart a bit, and with some other mechanisms it can reduce your blood pressure effectively. There is no withdrawal symptoms if your blood pressure has been returned to normal values.
Our blood pressure fluctuates all the time. No one really knows when will be the pressure goes up once again abnormally unless we find out the cause!
It is not very common for people to have a high blood pressure at your age, but sometimes it just happen and disappear for the rest of your life.
Please check if you are taken some other medication, even special food, that may affect your blood pressure sometimes.
If your blood pressure goes up agin, I would suggest you to have a full medication checkup in order to find out the cause of your high pressure.

Pill Scare

Right now I am using a contracetive call "Minulet (pills)". Due to the news report before, I would like to change another brand, could U give me some suggestion.

If you are worry about the cardiovascular side effects, I would suggest you to use the "Microgynon 30" by Schering Health (my wife is actually taking these!). Just wait until you have finish the cycle with Minulet, and start using Microgynon on the first day of the cycle. It is a medium dose of 2nd generation combined oral contraceptive, it has been use for
 quite a while. Before you switch to another brand, it is advisable to have a
body check up and consult the opinions from your GP.
Microgynon is safe and effective for most women, but if you are age over 35 ,or smoking, or having a history of cardiovascular disorders or obesity or diabetes or migraine, you should
consult your GP or the family planning nurse.

The mainly purpose using the contraceptive is that it can reduce the hormone in order to increase my weight and take the advantage of my skin. I would like to know that the contraceptive (Mircogynon 30), which  you advice it to me last time, is still have side effect for my fancy function.

Microgynon 30 is the second gereration oral contraceptive pill which contains no gestodene or levonorgestrel. It should also work for the skin (acne?)because it also suppresses the excessive androgen that causes the skin problem.But it is  not licenced for that condition, that means there is no clinical evidence for this use! There is a prescription medicine called Dianette specific for the women with androgen dependent skin condition. Becasuse the skin condition is a complicated disease, please ask the specilist for further advice.

Buspirone

    I was just looking through the website and when I clicked onto the "Smoking Cessation Treatment", it went to a geocities sign up for email page. Because, in Canada they have this new indication for buspirone  for people to stop smoking. It is marketed as Zyban. Have you heard?  I'm afraid that some people will take this as an easy pop a pill method  of quitting smoking. What do you think?

    I have heard about buspirone. It is called Buspar(by BM) in HK & UK and it is a Prescription Only Medicine. I believe it should not have any problem as long as it is POM and take under direction. Of course, the street market is always waiting for new-comers. Besides, most of the people who havedetermination to quit smoking, would not choose the street market. ( The insurance cover this newindication in Canada?). Do you know about its comparative efficacy to the currently marketed
products , such as Nicotine gum?

The error has been amended, It was due to a " smoking cessation Treatment" section was

     The fact is I don't know, because there are so many brand namesavailable nowaday!Some of them you may never heard of. What you  can do is try to check the generic (drug) name to see whether it is omeprazole 20mg, If it is the case it contents the same  material as losec. If you want me to guess, if it stated as 20mg and for the stomach, and it is expensive, the chance of losec equalvent is high!

Hepatitis vaccination
    I am a Canadian to visit Hong Kong for an extended stay. My doctor advises that my family of 4 should be vaccinated for     Hepatitis   A  and   Hepatitis B.
    He prescribed Havix 1440 for the adults and Havix 720 for the  children  in two doses taken over a 6 month period. He also prescribed  Eugerix B Vax 20 mg for adults and Eugerix B Vax 10 mg for the children in  3 doses  to be taken over a 6 month period.  Because of the extended length of stay, and because the  medications need   to be refrigerated, I am debating whether I should bring them  with me to  Hong Kong. Are these medications readily available in Hong   Kong ?
    If   yes, would you know the approximate cost ? Do I need a prescription from a HK doctor to obtain these medications ?
    If you are not able to provide answers to my questions, would you kindly refer me to someone who will. Thank you and I look forward to your response.

          If you and for family have not been vaccinated, I suggest    you to do so. (well the risk of getting infected isn't that high   in HK actually). Those vaccine are readily available in HK. Depend on how   long you will stay, If you stay more than half a year, I suggest    you to have  the blood tests for hap A -&- B ,and the fist dose in    Canada and then do the rest in HK. (If you take in the vaccine,    you may encounter some custom  problems).
          It will cost you about approx. $100-200 HK per dose in HK.  If your child is really young (say less than 10) ,and they have  HKID cards, I suspect they can manage the get the vaccines free  from some of the health centers in HK.
